IMPORTANT
INFORMATION FOR NEW PATIENTS
Federal law requires all healthcare practices
to obtain, verify, and record information that identifies each new patient.
What this means for you: When you open an account, we will ask for your
name, address, date of birth and other information that will allow us
to identify you. We may also ask to see your driver’s license or other
identifying documents.
Upon your first visit we will obtain your insurance
information. We are participating with many insurance plans, including
Blue Cross Blue Shield, Carolina Care Plan, Physicians Care Network and
Cigna. We also accept MasterCard, Visa, checks and cash. If you have HMO
coverage that requires preauthorization, please notify us as soon as possible.
We will help you in obtaining the preauthorization; you will be responsible
for your co-pay at the time of service.
The charges you incur are your responsibility; we will assist you when possible with any questions that may arise from your
insurance carrier. Our billing department is available from 8:30am-5:30pm Monday through Friday to discuss any concerns you
may have.
